PLYMOUTH- Mid-State Health Center is happy to announce its new PAR program.

What is it? PAR - Personalized Assessment and Recommendation visit for prevention and screening only. Acute or Chronic problems will not be addressed during this visit.

Who is it for? A PAR visit is for healthy individuals who are concerned for their health.

What does it include? A PAR visit includes a basic exam, screening tests and/or lab work based on your health history and risk factors, an explanation of your risks for certain chronic diseases (i.e. Heart Disease and Diabetes), targeted recommendations and a handout of resource information to help you take action.

When does it start? We are scheduling PAR visits starting March 5, 2008 with Dr. Mustapha Karanouh and Kim Fader, ARNP. These appointments will only be available on Wednesdays and Thursdays until we expand this to the other clinicians.

Additional Information: Patients will be asked to arrive 15 minutes before their scheduled appointment so that the nurse can complete all of the information for the clinician before the start of the visit. The total appointment time will be approximately 1 hour.
